The undersigned player/participant hereby confirms their desire for player/participant to engage in the Sunset Madness Basketball League. It is understood that injuries can be a common and ordinary occurrence during these types of activities. In return for being allowed to participate in the Sunset Madness Basketball League, the undersigned hereby agree to freely, voluntarily and expressly ASSUME and accept any and ALL RISKS of any injury to any part of the player/participant's body while attending or engaging in any portion of this clinic or its related activities, including but not limited to any injury or damages arising out of or relating to participation in any activities, or use of the facility or services we provide, or on account of any illness or accident, or damage to or loss of your personal property.
Participant hereby agrees to FOREVER RELEASE the Sunset Madness Basketball League, all its providers, board members, sponsors, and co-participants from, and agree to indemnify them and hold them harmless for, any and all responsibility or legal liability for any injuries or damages to the undersigned participant, whether or not such injuries or damages are caused by the NEGLIGENCE of the Sunset Madness Basketball League or any negligence of the owners or operators of the facilities where the clinic is to be held. Participant agrees NOT to make a claim against or sue the Sunset Madness Basketball League or any of its providers, board members, sponsors, and co-participants and accept full responsibility for any and all injuries and damages.
By signing this document, you acknowledge that you have carefully read this waiver and release and fully understand that it is a release of all liability. You agree to allow all photos and videos recorded during games to be used on Social Media and promotion purposes. In addition, you hereby waive any right that you may have, by or on behalf of yourself, your heirs or representatives, to bring a legal action or assert a claim for injury or loss of any kind. However, it is not intended to assert any claims or defenses that are prohibited by law. You agree that the foregoing agreement is intended to be as broad and inclusive as is permitted by law and that if any portion or paragraph is held invalid, the balance shall continue in full legal force and effect.
